Preparing for Your Visit

What to Bring

  • A valid government-issued photo ID
  • Your current insurance card
  • A complete list of medications, including dosages
  • Any recent medical records or test results

Arrival Instructions

Please arrive at least 15 minutes before your scheduled appointment to allow time for check-in and any required paperwork. Complimentary parking is available in the garage located behind the building. Be sure to bring your parking ticket for validation.

What to Expect
